If you would like to have your toll-free numbers ported to Aptela, you need to complete the required forms below. Aptela needs these documents to initiate the porting process:

1. Aptela Porting Request Form -  This form collects your Aptela account and contact information, as well as how your porting numbers should be assigned, or if Aptela should remove any temporary numbers from your account, once the port completes.
2. Form 1 – Responsible Organization Change Form - This form collects the toll free numbers you wish to port to Aptela and company contact information
   
3. Form 2 – Letter of Authorization for Toll-Free Number and Responsible Organization Transfer - This form also collects the toll free numbers you wish to port to Aptela and company contact information.
4. Copy of a recent bill from your current provider

Download and complete the forms for all the numbers you would like to port. Be sure to complete the required fields marked with an asterisk (*). You can fax these documents to (202) 747-2049 or email in PDF format to porting@aptela.com.
